*RN Care Manager*

Performs professional nursing activities according to evidence-based practice standards. Assumes responsibility for overseeing the primary needs of the patient (client), including assessment of the client and family/caregivers, establishment of the nursing plan of care, activation of interventions, coordination and collaboration with interdisciplinary team, and evaluation of outcome. Travels independently and provides skilled nursing care in the client's home.
Service Delivery:
Conducts initial and ongoing nursing assessment at required time points in an integrated, comprehensive assessment process
Monitors continuing status and identifies significant changes which impact care
Accurately documents and communicates assessment data, plan of care and nursing intervention within established agency standards
Prioritizes nursing care needs
Develops and documents nursing plan of care
Identifies barriers to treatment outcomes and adjusts plan of care accordingly
Implements nursing care according to the appropriate care guideline
Provides health education to promote client/caregiver independence in the management of health care needs
Coordinates client care and changing care needs with the physician and other team members
Demonstrates sound judgment in the practice of home care and hospice nursing
Provides and documents discharge planning which reflects involvement of the client/caregiver
Evaluates outcomes of activities performed during each visit
Evaluates treatment outcomes and makes revisions to the plan of care based on outcome evaluation as necessary
Initiates appropriate referral for other services as necessary to promote outcome achievement
Adheres to Medicare and other regulatory service and documentation requirements
Delivers hospice care which reflects principles of the impact of terminal illness, pain and symptom management, and end of life care
Assumes clinical leadership role in collaboration with the case supervisor, physician, and other team members to promote achievement of treatment goals and objectives
Implements strategies designed to support Relationship Centered Care
Practices strategies to support Avatar patient satisfaction
Efficiency:
Provides nursing services in an efficient, cost effective manner
Utilizes a case management approach which incorporates principles of responsible fiscal management in conjunction with the clinical needs of the client
Maintains daily visit quantity within established agency productivity standards
Professionalism:
Supports agency mission, vision and goals
Is aware of and adheres to the guidelines as stated in the Corporate Compliance Program of Borgess Health
Conducts self in professional manner
Follows agency policies and procedures
Maintains appropriate professional appearance

Teamwork:
Participates as a team leader/care manager using a collaborative style to identify and resolve problems related to clinical practice (nursing team, interdisciplinary team, intra/inter agency communications)
Conducts HHA supervision per agency policy and procedure
Participates in Gallup Impact Planning, designed to support employee engagement
Requirements:
Graduate of a National League of Nursing accredited school of nursing (Bachelor of Science preferred). One year in acute care and medical/surgical setting required (preferably in community health nursing). Hospice experience preferred. Must possess a current license to practice as a Registered Nurse in the state of Michigan. Must also possess a current driver's license, proof of current automobile personal/property liability insurance, and have reliable transportation. Must meet agency motor vehicle record standards. Current BCLS certification is also required. Basic computer skills or satisfactory completion of introductory computer training required.
