Is responsible to see that quality nursing care is provided with the assistance of other team members to all residents living in the assigne! d area maintaining Clark standards and practice.

Coordinates all aspects of each resident's nursing care with the assistance of other team disciplines for the time assigned with them.

Provides continuity of care in correlation with the other shifts over a 24-hour period.

Performs other related duties assigned by Director of Nursing or Neighborhood Clinical Care Coordinator.

Accounts for Narcotics CII and Controlled Substances CIII through CV with on-coming/off-going shift Charge Nurse.

Charge Nurses will be required to attend in-service training to improve and strengthen their knowledge related to caring for residents with dementia. Charge Nurses will be required to implement and apply knowledge gained through experience and training to provide a safe and caring environment for residents with dementia within established policies and procedures including but not limited to:

Observational Skills: demonstrate ability! to recognize, document, and report behaviors, moods, physical! characteristics, changes of residents.

Person Centered Care: demonstrate ability to provide personal care in a manner that is safe for the resident and is suited to their personal preferences. Utilization of validation techniques with residents who are upset, frightened or confused. Staff will be present with residents and involve them in activities that are appropriate for the resident's skill level.

Problem Solving/Critical Thinking: demonstrate the ability to process and resolve issues related to resident behaviors. Ability to recognize and respond to resident behaviors in an appropriate manner. Ability to identify and appropriately address challenging behaviors of residents as well as identify possible methods and approaches to minimize or eliminate challenging behaviors.

Planning & Organizational Skills: Demonstrate the ability to plan (with guidance) and implement care and activities (group & individual) in a manner whi! ch incorporates resident choices and preferences. Also maximizes the time spent with residents in providing meaningful interaction with residents beyond activities of daily living.

Communication Skills: Demonstrate ability to communicate with residents as well as understand the communication style of the resident. Ability to recognize verbal and non-verbal information communicated through behaviors and spoken communication. Demonstrates skill in approaching confused and demented residents to minimize catastrophic reactions and resistance to care. Speaks to residents and family members respectfully and courteously. Speaks to team members in a courteous and friendly manner makes positive and helpful comments.

Resident Rights

Is aware of resident rights and incorporates them into daily practice.

Knocks before entering resident's room.

Calls resident by preferred name.

Follows resident direction and choice whenever po! ssible.

Provides for privacy during resident care.

Resident Care

Understands Clark Policies/Procedures/Protocol.

Receives report about each resident in assigned nursing area, counts required drugs and receives keys for the area from the previous shift Charge Nurse.

Directs nursing care of each resident and sees that it is carried out by the appropriate team member.

Administers all medications per doctor=s orders and Clark Policies/Procedures/Protocol.

Performs all treatments for residents requiring attention or assists others to perform them as necessary providing needed instruction and supervision as required.

Interacts with all residents on assigned nursing area during assigned shift assessing each resident's condition to assure optimum care: Medical and nutritional needs, emotional, social and spiritual needs.

Makes sure standards of care are! met for each resident by their assigned team member.

Keeps Director of Nursing/Neighborhood Coordinator informed of all condition changes, environment needs, staffing and equipment needs.

Contacts other team disciplines when there is a need: Dietary, Activities, Social Worker, Rehab, PT, OT, Speech, Pharmacy or Physician.

Makes sure environment is safe and clean by contacting appropriate departments: Housekeeping or Maintenance.

Is familiar with all aspects of resident=s history or knows where to find the information: Present condition, past, allergies, treatments receiving from other departments (PT, Rehab).

Sees that care is delivered so as to assure safety for self, staff, and residents:

Follows Universal Precautions.

Uses transfer belts and other appropriate mobility assisting devices.

Is in contact with resident's physician per telephone or in facility to deliver needed care to resident.
Contacts families regarding condition changes, needs, or on requ! est of the resident.

Responds to emergency situations immediately with composure, competence and good judgment following Clark Policies/Procedures/Protocol while giving reassurance to resident and others in the area. (In case of injury, fire, condition change, etc.).

Sees that residents get to all scheduled or emergency appointments with the assistance of team members, (Dr. or family visits, x-rays, beauty shop, activities).

Performs assessments and specimen collection according to Clark procedure. (Vital signs, urine, stool, I & O, B & B, Assessment/ Retraining, admission assessment).

Works with team members to preserve skin integrity. (Keep skin clean and dry, prevent shear injury, and prevent decubs).

Interpersonal Interaction/Communication

Speaks to each resident respectfully and courteously as defined by resident.

Speaks to family members respectfully and courteously as defined by family members to create g! ood rapport.

Addresses team members in a courteous and friendly manner - makes positive and helpful comments.

Demonstrates skill in approaching confused and demented residents to minimize catastrophic reactions and resistance to care, medications, treatments. Gives complete, concise report to on-coming shift.

Answers telephone per Clark protocol.

Relates verbally to other departments in a courteous and friendly manner to solve problems.

Teamwork/Supervision

Takes direction from Director of Nursing/Neighborhood Coordinator in a pleasant manner, accepts suggestions from team members, offers suggestions to team members.

Has the ability to manage a team and problem solve, knows when to ask for help, takes initiative to problem solve as need arises.

Has the ability to recognize when a team members work does not meet Clark standards of care and is able to confront person in a private setting to provide constr! uctive criticism and problem solve in a courteous and helpful manner.
Is respected by team members for using fair and good judgement in making decisions which involve them.

Has the ability to manage the team in assigned nursing area in a way that produces a calm, cooperative atmosphere.

Able to change team assignments to assure optimum care for all residents.

Is willing and able to assist certified staff when necessary.

Documentation

Documents all condition changes and follow-up on situations from preceding shifts.

Documents on medication and treatment sheets per Clark policy on a daily basis.

Documents on all residents on Medicare daily until discontinued.

Checks to be sure team members document per Clark policy. (FA, I & O, B & B Assessment/Retraining, resident care record).

Completes written reports when accidents, errors, or other unusual incidents occur and documents on resident's chart.

Fills out monthly summaries and updates care plans as ! assigned.

Documents on all necessary department sheets in order to maintain a team approach to all problems. (Housekeeping, maintenance, Social Worker).

Attendance/Time Management

Works required days & required hours.

Does not leave work station early.

Manages break time: Does not extend breaks. Notifies Director of Nursing, Neighborhood Coordinator and team members when off unit of location where to be reached. Responds promptly to emergencies if called away from break.

Completes assignment in time allowed with minimal overtime.

Attends meetings and in-services to increase professional skills.

Physical Requirements:
Sits, stands and walks throughout resident care areas. Communicates frequently verbally and through written methods. Works in an office area that is smoke-free, well ventilated, well lighted and barrier-free. Position requires frequent walking, bending, stooping, twisting, walking, and bending and lifting.
