Position Overview:
Shared Services Home Health Care, a Medicare-certified home health care division of Evangelical Homes of Michigan, is excited and eager to welcome a new LPN Quality Assurance (QA) Nurse for our Ann Arbor, MI Home Care office, servicing the Washtenaw County and surrounding areas. Our LPN QA Nurse will be responsible for the quality of all clinical documentation and has accountabilities for the monitoring, implementation and evaluation of processes and programs to ensure continued excellence.
Responsibilities for the LPN, QA Nurse include:
- Assist managing all aspects of patient intake! process.
- Establishing and maintaining positive relat! ionships with customers and referral sources, and responding to customer requests and concerns.
- Assist with managing insurance verification and authorization process, as needed
- Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, assist clinicians to navigate the quality assurance process to promote more effective performance and delivery of quality home care services and is available during operating hours to assist clinicians as appropriate.
- Reviews patientâs clinical diagnosis, medications, procedures and planned clinical course for completeness, consistency and regulatory compliance.
- Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities and developing a plan of care as needed.
- Attends case conference meetings with organization personnel to facilitate coordination of care.
- Conduc! ts record reviews and communicates findings and recommendations to the Quality/Intake Manager and appropriate organization personnel.
- Assists the Quality/Intake Manager in the planning, implementation and evaluation of in-service and continuing education programs.
- Provides period on-call and field support, as needed.
- Current, active licensure to practice as a LPN in the State of Michigan
- A minimum of two years experience in quality assurance, preferably in a home care setting.
- Familiar with OASIS, working with CMS contracts, participation in State Survey and Audits
- Must possess excellent verbal and written communication skills, including active listening.
- Knowledge of government regulations, CHAP standards, Medicare Conditions of Participation and private payer practices.
