Position Overview: Provides nursing care to meet the needs of an assigned group of residents within the framework of the total plan of care. Functions under the direction of the Registered Nurse. Supervises various nursing personnel on a regular basis.

Essential Functions and Responsibilities: include the following. Other duties may be assigned.

Deliver nursing care that complies with federal, state, and local regulations.

Supervise delivery of nursing care by GNA’s on assigned shift and unit.

Ensure nursing care delivered by GNA’s is delivered with respect for residents’ rights.

Provide direct resident care; assist GNA’s with difficult assignments.

Documents residents’ responses and the effectiveness of nursing interventions, including the administration of PRN medications.

Administer resident medications according to the institution’s policies and pro! cedures, without significant errors.

Carries out rest! orative and rehabilitative programs, according to resident care/service plan.

Makes daily rounds on each resident and initiates concurrent review procedure when appropriate.

Admit, transfer and discharge residents as necessary.

Complete accident/incident reports as necessary.

Transcribe physician’s orders to resident charts, cardex, medication cards, treatment/care plans/service plans, as required.

Prepares monthly rounds sheets and participates in rounds.

Maintains confidentiality.

Displays a caring and respectful demeanor towards residents, families, and staff

Manages difficult or emotional customer situations

Rspond to all resident requests (verbally or call-light) for assistance.

Practice positive resident relations, responds to resident and family inquiries, concerns, or problems.

Perform technical aspects of resident care according to established policies and procedure! s. Adapts techniques to individual resident’s needs.

Administer medication with knowledge of action, route of administration, side effects and contraindications

Administer professional services such as: catheterization, tube feeding, suction, applying and changing dressings/bandages, packs, colostomy, drainage bags, ROM exercises, care for the dead/dying, etc., as required.

Satisfactorily completes annual competency check list.

Assumes responsibility for own professional growth and development as evidenced by participation in classes and seminars that facilitate enhancement.

Acts as charge nurse as assigned.

Contributes to resident assessment and MDS/Care/Service Planning process.

Communicates about residents’ condition between and among nursing staff.

Exchange resident information with nursing staff on an ongoing basis throughout shift.

Gives accurate and concise report to the follo! wing shift.

Report all allegations of abuse/misappropriation o! f resident property per nursing policies

Provides accurate written documentation on 24-hour report.

Plans, supervises and instruct other staff members work.

Explains nursing techniques and procedures and assists nursing personnel in providing resident care.

Displays a willingness and ability to work as a team

Maintains a positive attitude, does not offer complaints, offers suggestions.

Practices all safety & loss prevention measures; adheres to safety rules and regulations.

Demonstrates knowledge of emergency policies and procedures.

Offers assistance and support to co-workers

Demonstrates a time-conscious awareness in order to complete resident care priorities.

Verify the identity of residents prior to administering medications/treatments.

Utilizes gait belt appropriately.

Implement and practice infection control policies and procedures; ensure compliance by assigned! GNA’s to universal precautions and all other infection control guidelines; inform supervisor of any real or potential infection control concerns.

Effectively adjusts assignments with changes in resident care needs.

Provides leadership to nursing personnel assigned to that shift.

Participates in the orientation of new residents/families to the facility.

Informs charge nurse of any changes that need to be made to Care/Service.

Plan and initiates those changes.

Knowledge of when to seek help or guidance from immediate supervisor or appropriate resource person.

Provides education to families regarding disease process and adjustment to continuum care.

Maintains own staff development record accurately.

Regularly attends staff meetings; when unable to attend, reads & initials minutes.

Meets mandatory educational requirements as required by regulatory agencies.

Required documentati! on completed accurately and timely (monthly summary note, comfort calls! , quarterly & annual MDS, Braden scale, Falls Assessment, Care/Service Plans, skin rounds, restraints, MMDS, GNA Care Plans)

Signs and dates all entries made in resident’s medical record.

Meets attendance and punctuality guidelines.

Works well with supervising RN; open to constructive feedback.

Willingly attends in-service training.

Demonstrates willingness and ability to learn new policies and procedures

Schedules time off in advance.

Begins working on time.

Keeps absences within guidelines.

Flexible in meeting unit scheduling needs.

Works where needed without complaint.

Knowledge, Skills and Abilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience: Bachelor’s Degree (B.A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. Basic Computer skills required.

Supervisory Responsibilities: Carries out supervisory responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring and firing; training employees, planning, assigning, and directing work; appraising performance; coaching, counseling and disciplining employees; addressing complaints and resolving problems.

Language Skills:
License, Certificates, Registration: Licensed by State.
