Evaluates patients, plans and implements goal directed treatment plan for cases. Coordinates care and consults with nursing and rehabilitation staff.
DUTIES
1.Performs the assessment function for adult and pediatric clients. Assesses on an ongoing basis the psychosocial needs of patients and families based on emotional, environmental, financial, psychological, cognitive, social, cultural, and health factors. Provides short-term patient/family therapy when indicated by assessment.2.Identifies the availability of support systems and community resources. Initiates referrals to appropriate community resources and assists the patient and family in accessing resources. Assists the patient/family in applying for insurance benefits and other financial support.3.Collaborates with the patient/family/caregiver and the interdisciplinary team in the development, evaluation, and revision of the plan of care of the patient/family unit, including end of life planning and transition to hospice consistent with agency expectations of patient care.4.Documents care provided and the response of the patient/family in a risk-neutral fashion; recog! nizes the sensitivity of psychosocial data and protects confid! entiality of same; submits documentation which reflects third-party payer requirements in a timely fashion as described in agency policy focusing on agency results.5.Participates in the orientation of new members of the interdisciplinary team and students.6.Manages a caseload and delivers care in a cost-effective manner that demonstrates an understanding of home care reimbursement methodologies focusing on agency results.7.Seeks opportunities for professional growth, including demonstrating the ability to receive constructive performance feedback.8.Acts independently and responsibly to perform job duties on a consistent basis.Supplemental Job Functions: 1.Develops a proficiency in the use of laptop computers and applicable software to record patient assessments and notes.2.Continues to develop clinical skills to meet future professional requirements.3.Performs other duties as assigned.
QUALIFICATIONS
1.Master's degree in Social Work (MSW) from a school of ! social work approved by the Council on Social Work Education.2.One to two years of medical social work experience; home care experience preferred. 3.CSW/ACSW preferred.4.Current driver's license, reliable transportation, and current, valid automobile insurance.Required Skills and Abilities: 1.Must write, speak and comprehend English to effectively communicate with patients, staff and others.2.Must be able to hear normal conversation with or without electronic device assistance.3.Must have visual acuity correctable to 20/30.4.Must be able to climb stairs without assistance.5.Must have physical ability to provide patient services under normal and adverse or unpleasant conditions due to weather, environment or patient lifestyle.6.Must operate a motor vehicle in a safe manner and maintain a valid Michigan driver's license. 1.Performs the assessment function for adult and pediatric clients.
