DUTIES:
Tour of Duty: Primarily days, Monday thru Friday but may be required to work some weekends. Eight (8) hour shifts that may rotate between the hours of 7:30am and 8:00pm.

LPN will be assigned to the Patient Aligned Care Team (PACT). Incumbent will provide team based care to primary care patients along with providers, RNs, Clerks, and other staff. Patient care is provided in a multiple of ways including face to face care, telephone care, group visits, and secure messaging.

Provides direct support to the provider in assisting with care coordination by responding to view alerts, messages and faxes.

Provide patient and family education regarding patient's condition, procedures, and preventive health care.

Assists patients to identify and establish health goals using motivational interviewing techniques.

Makes astute observations and thoroughly documents care and the patient's response.

Appropriately seeks assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention.

Participates in prioritizing and as appropriate, modifying patient care treatment.

Performs assigned nursing care responsibilities requiring the use of advance equipment, technology, or knowledge in procedures performed.

Maintains precise adherence to basic principles of asepsis, safety and universal precautions to provide a safe environment for patients, families and employees.

Utilizes special equipment such as Doppler units, pulse oximetry, bladder scanner, EKG, point of care testing, and other instruments needed to perform the job duties.

Provides one on one nursing care to assigned outpatients whose care requires close observation during procedures.

Utilizes the computer to access patient data to verify appointments, review progress notes, and short medication profiles.

Orders labs and/or looks up test results under specific clinic profile.

Responsible for RME for all scopes and other equipment specific to their clinic assignment, and for cleaning non-reusable medical equipment between patients per standard operating procedures.

Physical Demands: This position may require prolonged standing and/or sitting for greater than 4 hours, may require lifting of 45 pounds, may require repetitive stooping, bending, kneeling, squatting, and/or carrying items from one area to another.

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
Citizenship. Citizen of the United States.

Education. Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or [accredited by] the National League for Nursing [Accrediting Commission (NLNAC)] at the time the program was completed by the applicant. Verification can be obtained from the State approving agency [or] the National League for Nursing [Accrediting Commission], 61 Broadway, 33 rd Floor, New York , NY 10006 . [Additional information may also be obtained from the NLNAC Website . Exceptions include:]

Licensure. Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico ) of the United States , or District of Columbia . An LPN[/LVN] who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of this part.

Physical Standards. See VA Directive and Handbook 5019.

English Language Proficiency. Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

GRADE DETERMINATIONS: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidateÂ's qualifications must clearly demonstrate the level of competence required for the grade.

Grade determinations will be made by the LPNPSB.

GS-3 - None beyond the basic requirements.

GS-4 - Six months of qualifying experience as an LPN or LVN; or Graduation from an approved school and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or Graduation from an approved school of at least 24 months duration.

GS-05 - Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.

GS-06 - Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN.

Qualifying experience is successful nursing practice [as an LPN/LVN,] maintaining acceptable standards within a health care setting under the direction of a registered nurse [(RN)] or physician [(MD/DO)].
