Evangelical Homes of Michigan (EHM) is eager to partner with a new Director of Discharge Planning and Case Management Services to j! oin in the continued mission and growth of our Evangelical Homes of Michigan â€" Saline healthcare and post acute rehabilitation facility.
In our Saline Community, we are dedicated to compassionate care, as well as providing care solutions to those facing a wide variety of medical concerns that require services provided by a residential care facility including nursing, social, recreational, and spiritual wellness. This includes long-term care, respite care, memory care, hospice care, and short-term rehabilitation.
EHM is looking for the right individual whose philosophies and passions align with our vision and mission, and will be inspired to lead the planning, development and direction of the overall operation of our Social Service department.

Essential Job Functions include, but are not limited to:
Develop, implement, evaluate, and direct the Social Service and Discharge Planning services, as well as its programs and activities, in accordance w! ith current rules, regulations, and guidelines that govern our! long term care unit and our post acute rehabilitations and wellness center.
Make written and oral reports/recommendations to the Executive Director, as necessary/required, concerning the operation of the social service department.
Develop methods for coordination of social services and discharge planning with other clinical and patient services to ensure the continuity of the clients’ total regimen of care, and that the medical, emotional and social needs of the client are being met/maintained on an individual basis.
Assist in the development of preliminary and comprehensive assessments of the social service needs of each client/resident. Ensure that all personnel involved in providing social services to the resident are aware of the resident’s care plan.
Participate in the discharge planning for residents/clients, and provide consultation and recommendations on continuing care services, Medicare/Medicaid and financial assistance programs as needed. Provide resident and family counseling, along with behavior management and coordination of services with the psychiatrist and attending physicians.
Determine appropriate staffing levels for social services department, and assist in the recruitment and selection of social service personnel.
Prepare and plan the social services’ budget and submit to the Executive Director for review, recommendations, and/or approval.

Qualifications and Requirements include:
Must possess a Bachelors Degree in Social Work, Bachelors Degree in Psychology or a Bachelors Degree in Nursing from an accredited college or university.
Must have, as a minimum, 2 years supervisory experience within Social Services or Discharge Planning in a healthcare facility or within post acute programs.
Must be knowledgeable of social service, case management and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
Mu! st possess leadership and supervisory ability and the willingness to wo! rk harmoniously with and supervise other personnel.
Must possess the ability to plan, organize, develop and implement the programs, goals, objectives, policies, and procedures that are necessary for providing quality care.
Must be willing to seek and welcome change and new methods, and be willing to incorporate them into existing practices.
Must possess strong written and verbal communication skills, including active listening.
