Description: Hometown Home Health & Hospice is presently looking of REGISTERED NURSE (RN) Home Health Care right now, this career will be placed in Michigan. Provides a variety of nursing services to clients in their place of residence in accordance with physician's authoriz! ed plan of care, currently approved professional nursing methods.
2. Assists the physician in assessing the patient's level of functioning and the development (and revision, as needed) of the plan of care by applying nursing theory and nursing process.
3. Knowledge and ability to apply accepted generalized nursing principles and practice to a broad range of disease/procedure specific situations.
4. Monitors the patient's medications, response to care or services provided, the actions or interventions taken, and the outcome of services provided.
5. Makes referrals to other available service options and community resources as appropriate.
6. Provides patient/caregiver home care services and education in reflecting compliance with plan of care, patient's rights and responsibilities.
7. When a certified home health aide is assigned, prepares written assignment, supervises, instructs and directs care in accordance with regulatory guidelines, Agency and ac! creditation standards.
8. Provides introductory visit on a! ide's first visit and completes supervisory visits at least every 14 days, thereafter.
9. Evaluates and regularly re-evaluates the skilled, intermittent home care needs of the patient; initiates, develops, implements and makes necessary revisions to the patient's plan of care. Assesses patient's continual care needs. Plan of care addresses all problems identified in the assessment or provides a rationale for not doing so.
10. Performs initial comprehensive nursing assessment on the date requested by the Agency, including OASIS items, in a timely manner according to Agency, regulatory, and accreditation standards.
11. Documents Start of Care and completes and delivers documentation within 24 hours of the visit.
12. Assures completion of plan of care (certification and re-certification) within timeframes outlined in Agency policies and procedures.
13. Establishes, teaches, and/or supervises patient/family members, home care personnel in individualized plan! of care.
14. Makes referrals to other disciplines as indicated by the needs of the patient or documents rationale for not doing so.
15. Observes signs and symptoms and reports to the physician and/or other appropriate health professionals as often as needed. Reports changes in the patient's condition.
16. Follows all Agency policies and procedures regarding service delivery, documentation, and care coordination.
17. Teaches, supervises and counsels patient and family regarding home procedures and other care needs as appropriate to the patient's condition. Utilizes agency educational material as appropriate.
18. Assures coordination and continuity of plan of care by maintaining liaison with other health care professionals assigned to the same patient. Attends patient care conferences, and initiates patient care conferences for complex and/or multidisciplinary patients whenever needed to insure good coordination of care.
19. Provides evening or weeken! d on-call services when required by agency.

Required Skills
Required Experience

1. License to practice as a Registered Nurse in the State of Michigan.
2. Graduation from an accredited Registered Nursing program.
3. Current Michigan Driver’s License.
4. Willingness to drive in all kinds of weather within agency service area.
5. Ability to work the hours required by the agency including evening and weekend hours as needed.
6. Health and stamina sufficient to perform the job.
