Reporting to: Case Manager/Clinical Coordinator
Status: Non-Exempt/ Part-time
Supervises: Paraprofessional Clinical Staff
GENERAL DESCRIPTION OF RESPONSIBILITIES
The Staff Nurse/LPN/LVN, under the direction of the Case Manager/Supervising Nurse is
responsible for the delivery of coordinated, comprehensive care to patients through the
activities of assessment/evaluation, and provision of direct nursing care within the policies and
standards of the Agency.
DUTIES
1. Implements a comprehensive, goal focused, car! e plan for each patient, as developed
by the Case Manager ! and approved by the physician, makes recommendations to
revise the plan as needed and makes updates in provision of care in accordance with
written orders from the physician.
2. Works to achieve patient/nursing goals an expected outcomes as developed by the
Case Manager.
3. Provides those services and/or treatments requiring substantial and specialized
nursing skill.
4. Counsels the family and client in meeting nursing and related needs.
5. Supervise paraprofessionals providing services.
6. Makes planned home visits to the patient for ongoing assessment, instruction,
supervision and provision of nursing treatments according to the Plan of Care.
7. Makes recommendations for referrals to other services as indicated.
8. Maintains continuous contact with the patient’s Case Manager to report the
patient’s response to home care, any changes in status, or possible need to
additional services.
9. Prepares and maintains! clinical documentation according to Agency policies and
acceptable nursing standards.
10. Follows infection control and safety precautions.
11. Follows plan for the patient’s discharge from service.
12. Informs the Case Manager/Supervising Nurse of patient services provided and
needed and any changes in the Plan of Care.
13. Participates in orientation, in-service programs, staff meetings, and case
conferences.
14. Maintains a working knowledge of third party reimbursement in relation to home
health patient benefits.
15. Recognizes legal responsibilities involved in patient care, respects patient rights, and
property.
16. Contacts the Case Manager/Supervising Nurse to inform him/her of alternate

schedule for patient visits during vacation, sick days or personal days off. Reports
any schedule change or inability to complete assignment as scheduled.
17. Acce! pts changes/additions to schedule during staffing difficulties when nec! essary.
18. Organized daily visit schedule to minimize travel time and mileage. Maintains
productivity according to work status.
19. Performs other duties as assigned or needed.
CRITICAL DEMANDS
1. Visual/hearing ability sufficient to comprehend written/verbal communication.
2. Ability to perform tasks involving physical activities, which may include heavy lifting.
3. Ability to deal effectively with stress.
QUALIFICATIONS
1. Licensed, registered nurse in the State of Michigan.
2. Minimum of 1-2 years home care experience.
3. Minimum of 1 year of supervisory experience.
4. BSN Preferred.
5. CPR preferred, not required.
6. Pay based on coverage area, experience, performance.
