We are seeking an experienced Hospital Director of Case Management. Must have Michigan RN license, Masters in Nursing/Business administration with (5) Five years of recent hospital Case Management experience.
Minimum of five (5) years clinical nursing and five years recent hospital case management experience, preferably in a management role. Advanced communications and interpersonal skills with all levels of internal and external customers. Knowledge of computer software and hardware applications. Ability to work with sensitive and /or confidential information. High degree of professionalism.
Knowledge of MIDAS and EPIC preferred and Proficiency in MS Word and Excel.
Our hospital is looking for a Director of Case Management. Responsibilities include direct departmental operations to ensure the delivery of safe, high quality, age-appropriate, cost effective care and/or service, as well as developing and implementing department priorities that enhance the organization's strategic plan, achieves agreed upon clinical and financial outcomes, and specifically advances evidence-based practice and patient safety.
Development and organizes/directs an integrated, hospital-wide case management program which monitors and directs action to ensure achievement of appropriate levels of hospital resource utilization to reduce costs including utilization, discharge planning and care coordination. This function is achieved through concurrent data collection, analysis and coordinated documentation and discharge focused activities.
