Seasons Hospice & Palliative Care is seeking a Dietician to join our team of professionals serving the Madison Heights to Detroit to Ann Arbor to Flint area one or two days per we! ek as needed. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care.
The Dietician provides dietary/nutritional counseling to the patient and family in accordance with the interdisciplinary plan of care.
Responsibilities
1. Reviews the nutritional assessment done by the hospice nurse as requested by the Team Director.
2. Visits the patient and performs a nutritional assessment if necessary, in accordance with the interdisciplinary plan of care.
3. Provides dietary counseling to meet specific patient’s needs in accordance with the interdisciplinary plan of care. Counseling must be relevant to the patient’s needs rather than the personal needs of the family. However, dietary counseling may be provided to family members to enable them to prepare food for the patient.
4. Attends patient care conferences as necessary.
5. Document! s all services to hospice patients and families.
6. P! erforms any other duties as assigned by the Team Director and as appropriate.
Qualifications
1. Registered Dietitian currently licensed by the state(s).
2. Minimum of one year of experience in a health care setting.
3. Demonstrated commitment to hospice philosophy of care.
