Job Summary: Coordinates the care for a group of patients as it relates to disease management, resource utilization, access, discharge planning, quality, service and efficiency across the continuum. This is an int! erventional role maintaining a patient centered focus.
Utilizing the nursing process & critical thinking skills, coordinated the care for assigned patients. Utilizes established criteria to identify and facilitate the appropriated level of care for assigned patients. Educate patient, family and health care team members regarding appropriated level of care.
In collaboration with patient, family, physician and other health care team members formulates and facilitates the discharge planning activities of assigned patients.
Coordinated care and monitors the utilization of resources for patients, physicians and third party payers.
Collects and aggregates data via Maxsys automated case management system.
Serves as an educator as related to the Care Management process to patient/ families and other health care team members.
The Care Coordinator job encompasses the entire continuum of care internal and external to the acute care! setting.
Extensive knowledge base related to third p! arty payers, placement and community resources with the local continuum of care.
Ability to work in a fast paced, ever changing environment with the ability to multi-task.
Requirements:
RN with Bachelors in Nursing, 3-5 years hospital nursing experience. Prior Case Management experience or similar role.
