Position Summary !
Responsible for managing patients and coordinating the services provided by multiple disciplines within and outside the health system. Maintains a collaborative practice agreement with specific physician(s). Exercises independent judgment in the assessment, diagnosis and initiation of selected protocols/procedures. Provides educational support for patients/significant others, health team members and the community and coordinates QA/QI studies to measure outcomes of care.
Qualifications
Basic Qualifications:
Education - Nurse Practitioners who obtained their education and certification after 2000 must show evidence of completion of a master’s, post-master’s or doctorate from a Nurse Practitioner program that is accredited by the Commission on the Collegiate of Nursing Education or the National League for Nursing Accrediting Commission
Experience - 3 years of experience in current, relevant clinical experience,Previous experience ! functioning in a collaborative role
Licenses - Registered Nurse (RN)*\\All\\
Nurse Practitioner â€" Licensed (NP)*\\All\\
Basic Life Support (BLS) Note used for BLS,CPR & BCLS)\\All\\
Advanced Cardiac Life Support (ACLS)\\All, except Neonatal Services and Hospice\\
Completion of advanced wound care course \All, except Neonatal Services\1 Year\
Certification in wound care management (as appropriate for patient population)\All, except Neonatal Services and Hospice\1 1/2 Yrs\
Completes credentialing and privileging process upon hire and undergoes reprivileging every 24 months or as determined by policy. \All\\
Neonatal Resuscitation (NRP)\\Neonatal Services\\
Drivers License â€" MI (MI DR LIC)*\\Hospice\\
Preferred Qualifications:
Experience - Experience as a Nurse Practictioner
Licenses - Certification in hospice and palliative care\Hospice\\
