Our organization has positioned itself for a future filled with opportunities to serve an even greater number of individuals in a variety of ways, including an array of home-based services and programs.Position Overview: Shared Care Services, a private-duty home health care division of Evangelical Homes of Michigan, is excited and eager to welcome a new LPN Field Nurse for our Ann Arbor, MI Home Care office, serving the Washtnaw County and surrounding areas. Our LPN Field Nurse will be responsible for providing compassionate, caring services to our home care clients, in accordance to the mission, core values, and standards set by Evangelical Homes of Michigan.Responsibilities for the LPN Field Nurse include: •Routine visits to home care patients.
- Work compassionately and patiently one-on-one with patients to assess and identify needs. •Develop and initiate patients ’ plan of care. •Communicate with physicians, patients and families regarding care plan and treatments. •Document patient assessments utilizing our computerized, remote charting.
- Develop discharge planning. Requirements for the LPN Field Nurse include: •Current, active LPN license in the State of Michigan •A minimum of 2 years experience working in home care and/or ER ICU. •Strong organizational and communication skills (written and verbal), including active listening. •Ability to effectively problem solve in a independent environment, while providing quality and compassionate care to our home care clients.Evangelical Homes of Michigan is excited to offer competitive wage and benefit packages, along with a fast paced working environment.
