Position Summary
Initiates, provides and coordinates individual! patient services and participates in the overall maintenance ! and evaluation of quality coordinated home care/hospice services.
Qualifications
Basic Qualifications:
Experience - 2 years of experience in relevant clinical nursing related to area of practice
Licenses - Registered Nurse (RN)*\
