Collaborates with the Medical Director and Care Management Services Manager to expedite patients’ correct level of care, services and financial considerations for the appropriate length of stay. Supervises Care Management staff in collaboration with the clinical care team to expedite the patient’s care analyzing cost, quality and utilization to facilitate the admission, continued stay and discharge of the patient by combining the objectives of established clinical pathways and utilization management standards. This position provides ongoing support and expertise to the care management staff and serves as a resource to meet individual patient needs. The overall goal of the position is to supervise the facilitation and promotion of interdisciplinary continuity of care/discharge planning and cost effectiveness through the integration and functions of care management staff and utilization of resources.
Minimum Job Requirements:
Education: Bachelor’s Degree in Nursing or health-related field
Experience: Two years previous discharge planning and/or directly related Case Management experience within last 5 years required. Knowledge of InterQual criteria. 2 years clinical experience in the assigned area preferred. Knowledge of community agencies for post acute-support, (medical and/or financial) preferred. Licensure/Certification: Registered Nurse in the State of Michigan and/or licensed MSW (LMSW). Case Management certification preferred (CCM and/or CHCQM).
Skills and Abilities: Excellent communication (oral and written) skills to insure patient, family and team satisfaction. Interpersonal skills necessary to negotiate with staff, families, patients and physicians. 1 year supervisory experience preferred.
