Description: LENAWEE COMMUNITY MENTAL HEALT is at present looking to employ INTEGRATED HEALTH NURSE right now, this position will be placed in Michigan. Full Time Integrated Health Nurse: Responsibilities include: identifying consumers who have chronic health conditions, make recommendations for health outcomes for inclusion! in the Individual Plan of Service; review lab results; perform health assessments. The Integrated Health Nurse works with a team of nurses to complete skilled nursing tasks for consumers receiving services through the Medication Clinic and provide clinical expertise, coaching and training to staff. Requirements include an RN degree, along with experience with psychiatric nursing. Bi-lingual candidates are encouraged to apply.
